Waiver Service Coordinator/Care Manager: Hancock County

We are seeking a dynamic and compassionate Waiver Service Coordinator/Care Manager to join our dedicated team. Remote office position within employee's home, and Individual's Home Visits within Hancock County. Home office is located in Lima, Ohio. In this vital role, you will coordinate and manage waiver services for individuals requiring specialized support, ensuring seamless access to community-based care options. Minimum Qualifications:

  • Current RN or LSW licensure in the State of Ohio required

  • At least one year of experience in long-term care, home and community-based services (HCBS) for elderly and/or disabled individuals, medical social work, geriatrics/gerontology, or a related field based on program requirements.

  • Must be able to adapt to changing job duties, be flexible, and have exceptional organizational skills.

  • Knowledge of HIPAA-compliant documentation and healthcare/social service systems.

  • Proficiency in Microsoft Office (Word, Excel, Outlook) and ability to learn data systems.

  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines.

  • Excellent written and verbal communication skills.

  • Ability to work both independently and as part of a team in a fast-paced environment.

  • Demonstrated commitment to person-centered and trauma-informed approaches.

  • Ability to accurately enter and maintain data across multiple systems.

  • Ability to pass BCII background check and pre-employment drug screening.

  • Ability to adapt to changing job duties, demonstrate flexibility, and maintain strong organizational skills.

 

Work Environment and Physical Environment:

· Office and community-based work environment. Remote home office with local travel to individuals on caseload.

· Frequent computer use and communication via phone and electronic systems.

· Ability to sit, stand, and perform repetitive tasks; occasional lifting up to 20 pounds.

Essential Functions:

  • Maintains regular contact with individuals, informal caregivers, and service providers to ensure services are appropriate, high quality, and meet current needs. Verifies services are delivered according to the person-centered service plan and modify plans as needs change.

  • Manages a caseload and completes required and as-needed home visits and contacts to assess program eligibility, ongoing service eligibility, and continued person-centered service plan needs.

  • Conducts comprehensive reassessments at least annually, or more frequently as needed (event-based), to evaluate changes in needs and appropriateness of care.

  • Monitors individuals’ progress toward established goals and objectives outlined in the person-centered service plan.

  • Provides individuals with information about community and agency services and makes appropriate referrals on their behalf.

  • Participate in care conferences with formal and informal providers.

  • Acts as an advocate for individuals and participates in state hearings as required.

  • Maintains accurate, detailed, and timely documentation in electronic systems in compliance with agency policies, HIPAA regulations, and program requirements.

  • Participates in training, team meetings, and professional development activities to support continuous learning and adaptability.

  • Perform other duties as assigned in support of organizational priorities and evolving community needs.
