Care Management Supervisor

Minimum Qualifications

  • Current RN, LSW, or LISW licensure in the State of Ohio.

  • Minimum of one (1) year of paid supervisory experience.

  • Minimum of five (5) years of paid clinical experience in Home and Community-Based Services (HCBS), long-term care, home health care, medical social work, or geriatrics/gerontology.

  • Genuine interest in the mission of Area Agency on Aging 3.

  • Valid driver’s license, reliable transportation, and automobile insurance meeting Ohio state requirements.

  • Must have reliable telephone access.

  • Proficient in computer skills and ability to learn required software systems.

  • Ability to pass a BCII Criminal Records Check (fingerprinting) and pre-employment drug screening (at agency expense).

  • Responsible for maintaining all required professional licensure (RN, LSW, LISW), including renewals throughout employment.

Essential Functions

  • Administer and enforce management practices, policies, and procedures.

  • Provide leadership, supervision, and support to Care Management staff.

  • Initiate and implement quality improvements, problem resolution strategies, and departmental innovations.

  • Provide training, guidance, and ongoing coaching to staff.

  • Ensure compliance with HIPAA and all applicable privacy regulations.

  • Prepare documentation and represent the agency in CDJFS State Hearings, as needed.

  • Participate in legislative visits and advocacy efforts on behalf of the agency.

  • Monitor and support staff performance and program outcomes.

  • Maintain flexibility in work location and schedule as determined by leadership.

  • Report directly to the Vice President of Program Operations, Ex. Vice President, Chief Operations Officer, or Chief Executive Officer.

  • Perform other duties as assigned by agency leadership.

Required Trainings

  • Program certifications required as needed

  • Training in all required computer software systems  

  • Any additional training required by the Vice President of Program Operations, and/or company.
