This role provides coordination, administrative, and programmatic support to advance an integrated service delivery model across Population Health programs. This role supports care coordination, health promotion, nutrition, Elder Justice, caregiver support, and related services, by ensuring efficient workflows, accurate documentation, and seamless communication across teams and community partners.
The Specialist plays a key role in supporting person-centered, trauma-informed services for older adults by assisting staff, maintaining compliance standards, and helping connect individuals to appropriate services and resources.
MINIMUM QUALIFICATIONS:
High School Diploma or GED required, Associate degree in social services, healthcare, or related field preferred.
1–2 years of experience in administrative support, healthcare, human services, or aging services preferred.
Basic proficiency in Microsoft Office (Word, Excel, Outlook) and ability to learn internal data systems.
Strong organizational skills with the ability to manage tasks, prioritize workload, and meet deadlines.
Effective written and verbal communication skills.
Ability to work both independently and as part of a team in a demanding environment.
Ability to accurately enter and maintain data across multiple systems with mindfulness.
Basic knowledge of HIPAA guidelines and confidentiality requirements.
Demonstrated commitment to person-centered and trauma-informed approaches.
Must have a valid driver’s license, reliable transportation, and insurance meeting state requirements.
Must have access to a telephone.
Ability to pass BCII background check and pre-employment drug screening.
Interest in and commitment to the mission of the Area Agency on Aging 3.
ESSENTIAL FUNCTIONS:
Provide administrative and programmatic support across Population Health programs, ensuring organized, efficient, and timely operations.
Support the Director of Population Health in coordinating and aligning workflows across Nutrition, Health Education, and Connected Care programs.
Assisting the program leads with cross-program coordination, communication, and implementation support.
Facilitate service coordination activities, including referrals, tracking, and follow-up, to support timely and person-centered service delivery.
Serve as a central coordination point for program staff by supporting communication, documentation, and workflow efficiency across teams.
Communicate effectively with individuals, caregivers, service providers, healthcare entities, and community partners to support integrated service delivery.
Maintain accurate, detailed, and timely documentation in electronic systems in accordance with agency policies, HIPAA regulations, and program requirements.
Support data entry, tracking, and reporting activities to ensure data integrity across Population Health programs.
Assist with compliance monitoring, audits, and documentation reviews to ensure adherence to regulatory and quality standards.
Identify opportunities for process improvement and support implementation of workflow enhancements across programs.
Provide general administrative support, including correspondence, document preparation, call routing, and record maintenance.
Participate in training, team meetings, and development activities to support continuous learning and adaptability.
Perform other duties as assigned in support of Population Health priorities and evolving community needs.
