PROGRAM SUPPORT SPECIALIST
MINIMUM QUALIFICATIONS:
Must have Internet/WiFI at telework station
High School diploma or GED. Associates or Bachelor’s Degree preferred.
Ability to operate a computer to perform essential job functions including: proficient typing skills and knowledge of Microsoft Windows and Microsoft Office, primary Word, Excel and Outlook. Power Point and Publisher a plus.
· Genuine interest in the Mission of Area Agency on Aging 3.
Proficiency in using computers and working in various software programs simultaneously.
Ability to provide excellent communication and interpersonal skills.
Ability to maintain confidentiality and comply with HIPPA standards.
Ability to establish and maintain effective working relationships with staff, providers, members, funding sources, and other agencies.
Ability to follow written and oral instructions, and to communicate effectively both orally and in writing.
Strong organizational and time management skills.
Ability to work independently and as part of a team.
Attention to detail.
Ability to manage multiple tasks and projects effectively.
Attitude conducive to Agency Mission and goals.
Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.
ESSENTIAL FUNCTIONS:
Service providers, social service organizations, County Department of Job and Family Services, physician’s offices, hospitals, and nursing homes and accurately relays written and verbal information.
Performs several telephonic, scheduling, and resource functions that ensure appropriate documentation, and/or providing general information, providing following up on contact calls and messages.
Perform functions related to
Coordinate services with providers, vendors, etc.
Providing and maintenance of individuals’ records and processing of requests for records.
Maintaining confidential case records that may include client data, documentation of services in a physical and electronic record client tracking system and completes related daily, weekly, monthly, or yearly information as required.
Adheres to quality policies, procedures, and plans per internal and external agencies.
Completes support duties to include creating letters, forms and reports.
Documents all individual, CDJFS, physician, and provider contacts when appropriate in individual’s record.
Maintains accurate and up-to-date records within electronic storage system.
Adheres to privacy and HIPAA regulations.
Serves as liaison between the Community Options Program, Community Options Individuals, and the CDJFS.
Record Eligibility information in Ohio Benefits to include waiver approvals, monitors Medicaid status, process disenrollments and enter LOC’s.
Follow up with collection of the outside information needed to keep Medicaid approved and active for Community Options Individuals.
Track Medicaid eligibility to keep Community Options personnel aware of an individual’s financial eligibility for Community Options and other programs.
Assist Community Options personnel, individuals, and individual’s families in understand Medicaid requirements and regulations.
Process official notices for Medicaid approval and liability amounts.
Adheres to privacy HIPAA regulations.
· Work/job performance location and scheduled hours will be determined by immediate Supervisor/Vice President/Chief Operations Officer/Chief Executive Officer.
This position is directly responsible to the Home Care Program Support Specialist Supervisor.
Performs other related tasks as assigned by the Home Care Program Support Specialist Supervisor/VP Aging in Place/Sr. Ex VP/Chief Operations Officer/Chief Executive Officer.
