DEFINITION:
This position will coordinate health education activities related to healthy aging and disease prevention programs. The Health Educator will assist in developing and implementing health education outreach and engagement strategies, health education materials and group education sessions. The incumbent for this position will play a key role in planning, developing, and implementing health education training and prevention programs, as well as evaluating training strategies and materials. A primary function of this position is to serve as a health communication content expert and advisor related to healthy aging and other health related issues.
MINIMUM QUALIFICATIONS:
• Bachelor’s degree in Health education, Public Health, Sociology, Social Work preferred but not required.
• Highly effective communicator, both verbally and in writing.
• Energetic, enthusiastic, excellent organizational and leadership skills, and the ability to work with minimal supervision.
• Genuine interest in the Mission of Area Agency on Aging 3.
• Experience working with diverse and/or underserved populations (as employee or volunteer); and/or working with community-based organizations, public health agencies, and/or health care facilities.
• Excellent public speaking skills.
• Knowledge of general health promotion topics and primary and secondary prevention of specific chronic conditions such as diabetes, heart disease, obesity, and others.
• Ability to manage multiple tasks simultaneously, to stay organized, and to prioritize work appropriately.
• Proficient in computer skills. Intermediate competency in Microsoft Office Suite programs (Word, Excel, PowerPoint; Access), internet browsers, and email programs.
• Must have a valid driver’s license, access to an automobile, and automobile insurance meeting the minimum requirements of state law.
• Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.
• It is the sole responsibility of the employee to keep up with all required licensure (Examples RN, LSW, RD), including continued licensure renewals throughout your employment with Area Agency on Aging 3.
ESSENTIAL FUNCTIONS:
• Develops and provides education and outreach on health aging and chronic disease prevention topics in collaboration with community partners.
• Delivers health education and outreach programs at various community venues.
• Conducts periodic assessments of community interests and health education needs in areas of health promotion, disease prevention and maintenance, in collaboration with community and academic partners.
• Develops, adapts, and implements education materials and presentations to be culturally appropriate for specific populations.
• Establishes and maintains rapport and strong working relationships with community partners, attends community meetings at various times.
• Collects and maintains program monitoring and evaluation data on outreach and education activities and prepares periodic reports of outreach and education activities.
• Recruits, trains, and coordinates Lay Trainers and Coaches for agency education programs.
• Develops, implements, and coordinates various health education activities involved in providing national and local information and educational programs for Area Agency on Aging 3.
• Adapt and implement national and local information, campaigns, and education programs to meet the needs of high-risk populations in the service area.
• Maintain interface with civic groups and health service agencies; develop health goals and determine availability of health services for cancer patients.
• Prepare reports, analyze adverse trends, and appropriate recommendations or conclusions; maintain accurate records reflecting work activities.
• Ensures the fidelity of evidenced based programming and conducts fidelity checks of Lay Leaders as able.
• Work/job performance location and scheduled hours will be determined by immediate Supervisor/Vice President/Chief Operations Officer/Chief Executive Officer.
• The position is directly responsible to the Population Health Director/VP of Community Living Services/Chief Operations Officer/Chief Executive Officer.
